Information submitted through the secure patient portal is not treated as ordinary public Website information. It may constitute protected health information and is handled in accordance with applicable law and Mentality’s Notice of Privacy Practices.

Please use the secure patient portal rather than the general Website contact form to submit sensitive health information when instructed to do so.
