2. No Emergency Services
The Website is not monitored continuously for emergencies.

3. Therapy Services
Mentality PLLC provides mental-health services to adolescents and adults, subject to clinical appropriateness, availability, applicable law, and the practice's intake and consent procedures.

Not every service is appropriate for every individual. Mentality may determine that a different level of care, provider, or emergency service is more appropriate based on clinical circumstances.
4. Telehealth
Mentality may offer telehealth services.
Telehealth allows services to be provided using electronic communications and may be appropriate for many patients but is not appropriate for every situation.
Before receiving telehealth services, patients may be required to complete applicable consent, intake, and technology requirements.
Patients receiving telehealth services should be physically located in a jurisdiction where the clinician is legally authorized to provide the service at the time of the appointment.
Patients should provide accurate information about their physical location when requested.
If a clinical situation requires in-person evaluation, emergency intervention, or a higher level of care, Mentality may recommend or facilitate referral to an appropriate service.
5. Scheduling
Mentality may use an internal scheduling system and/or designated scheduling technology.
Submitting a scheduling request does not necessarily mean that an appointment has been confirmed.
An appointment is considered confirmed only after Mentality or its scheduling system provides confirmation.
Patients should not submit confidential clinical information through an ordinary scheduling request unless specifically instructed to do so.
6. Patient Intake
Mentality uses Ensora Health / TheraNest for patient intake and related administrative and clinical-support functions.
Patients may be directed to the secure patient portal to complete intake paperwork, consent forms, questionnaires, insurance information, and other documents.
Information submitted through the secure portal may become part of the patient's healthcare record and may be subject to HIPAA and other applicable privacy protections.
7. Insurance
Mentality may accept certain health insurance plans.
Insurance participation and coverage can vary by clinician, plan, service, location, benefit design, authorization requirements, and other factors.
Verification of benefits is not a guarantee of payment.
Your insurance company determines whether a service is covered and how much it will pay.
Patients remain responsible for amounts that are their responsibility under applicable law, their insurance plan, and Mentality's financial policies, including applicable deductibles, copayments, coinsurance, and non-covered services.
Patients should contact their insurance plan directly for questions concerning benefits, coverage, deductibles, network status, and claims.
8. Self-Pay
Mentality may offer self-pay services.
Self-pay patients are responsible for applicable fees in accordance with Mentality's current financial policies.
Patients may request information about current fees, payment arrangements, cancellation policies, and other financial terms before receiving services.
If you choose not to use insurance and instead pay privately, additional legal rights or requirements may apply depending on the circumstances.
9. Good Faith Estimates
When applicable, Mentality will provide Good Faith Estimate information as required by federal law, including applicable requirements for individuals who are uninsured or who choose not to use insurance.
A Good Faith Estimate is an estimate and is not necessarily a guarantee of the final amount charged.
Additional services, changes in treatment, missed appointments, or circumstances not reasonably foreseeable at the time of the estimate may affect the total cost.

Mentality may use social-media platforms for educational, informational, community, and business purposes.
Following, liking, commenting on, or otherwise interacting with a Mentality social-media account does not create a therapist-patient relationship.
Mentality will not publicly identify an individual as a patient or disclose confidential patient information through social media without authorization, except as permitted or required by law.
Social-media platforms are controlled by third parties and are governed by their own terms and privacy policies.
12. No Guarantee of Results
Mental-health treatment can be beneficial, but outcomes vary.
Mentality does not guarantee a particular treatment outcome, recovery outcome, symptom reduction, or other result from therapy or other services.
13. Third-Party Services
Mentality may use third-party technology and service providers, including Squarespace for Website hosting and Ensora Health / TheraNest for patient intake, patient-portal, scheduling, practice-management, and related functions.
